Do you want to join the BlueLIFE Wellness Initiative?  If the answer is YES, you’ve found the right site.

Behavioral Science Services and the COA Board developed BlueLIFE as a way of helping sworn and civilian commanding officers improve and maintain their health. BSS is the program administrator.  That means we create and maintain this website, arrange health screenings, develop training materials, and provide nutritional counseling and health ‘nudges’ to help you achieve your personal health and wellness goals.  We also monitor program participation and help ensure cash incentives are distributed.  Because the program is under BSS, we are able to protect your personal health information and keep it completely confidential.

To join the program, BSS needs two things from you initially: your signed Informed Consent and a copy of your Personal Pledge.

Here’s what to do: Please click on each of the buttons below and (1) print out the two documents and review them; (2) sign them; (3) keep a copy for your records; and (4) return the original Informed Consent and a copy of the first (cover) page of the Personal Pledge to BSS.

You can scan and email these materials to us at –, fax them to BSS at 213-482-9596, or mail them to BSS, 221 N. Figueroa Street,Suite 650, Los Angeles 90012.  If you have any questions please contact Dr Kevin Jablonski at 213-486-0790.
